1. Name of your band, its members and the instruments they play
2. Mailing address
3. Telephone number
4. Email address
5. A description of your band’s musical style, distinctive qualities, noteworthy performances, and recordings
6. One CD containing music samples of your band
7. Dates (Saturdays) your band is available to play
8. Photograph(s) of your band suitable for publicity
9. Website URL for your band
10. Website URL where at least one of your songs can be played online
